Add pagination to mdcat.

Need to take care to omit incompatible escape codes like iTerm jump marks or OSC links (these may work with less -r/less -R though).

Optionally paginate mdcat's output

The new flag --paginate disables all specific features of terminal
emulators and pages the output through less -R.

Look at $MDCAT_PAGER and $PAGER for an alternative pager command to use.
