Replace the Context uber object with a clearly defined rendering state and write down all state transitions explicitly.

Track rendering state explicitly

Replace the old god-like Context object with a stack of clear defined
rendering states and explicitly define all transitions applicable for
Markdown events.

This vastly simplifies the rendering algorithm and fixes many long
standing rendering issues.  It also lays the foundation for future
improvements like proper text wrapping.
