Create survey/form selector component

We use the survey /or form selector component all over the dashboard to determine against which survey an action is to be taken, e.g. retrieve form results, view datapoints on a map etc. This component is the same in the different parts of the dashboard so we will implement it as a single component that is reusable. We will replace the survey/form selector in the assignments creation page.
