Standard Dialog box for web

I'm currently working on the final mocks for the Practice Session experience project, and this project has Dialog boxes, but we don't have it on the Design Guide. So I've designed a dialog, but we should be consistent with this component across the platform. It's most likely that this project will be completed before we have a standard dialog for it, but I'm filing this issue so this can be updated later, if needed.

Additionally, I'm using the same standard dialog box that I've used for the Lesson Player + Onboarding project, so we should be consistent with that as well.

Note: I am marking this as high-priority because we are looking into implementing these mocks for GSoC and they use the dialog component. (Under "Dialog for logged-in users" there is a note asking developers to clarify the correct component to use.)
